Intravenous Thrombolysis Is Associated With Better Outcomes in Large‐Vessel Occlusion Requiring Endovascular Therapy

چکیده
Background Intravenous thrombolysis (IVT) and endovascular therapy (EVT) are both important treatments for large‐vessel occlusion stroke. However, it is still unclear how the timing of IVT, EVT, need transfer a patient to an stroke center EVT affect outcomes. In this investigation, we study interaction between rapidity among patients with Methods This investigation analysis OPUS‐REACH (Optimizing Use Prehospital Stroke Systems Care–Reacting Changing Paradigms) registry from 9 centers in United States. Using database, extracted baseline characteristics patients, whether received time intervals patients’ care. Patient demographics were compared 2 groups using χ test categorical variables 2‐sample t ‐tests or Wilcoxon rank‐sum tests continuous variables. Multivariable logistic regression was performed determine adjusted associations 90‐day dichotomized modified Rankin Scale outcome. Results A total 1171 included final analysis, 38.9% had good functional outcome at 90 days. Male sex lower initial National Institutes Health score nonmodifiable factors associated clinical We saw no differences underwent primary secondary transport. On multiple variable receipt IVT only modifiable factor found overall effect last known well on outcomes unless IVT. Conclusions independently improved days odds ratio 1.51. Neither shorter nor direct transport versus therefore conclude that prehospital algorithms must account timely administration over EVT.

Intravenous thrombolysis and endovascular therapy for acute ischemic stroke with internal carotid artery occlusion: a systematic review of clinical outcomes.
BACKGROUND AND PURPOSE Strokes secondary to acute internal carotid artery (ICA) occlusion are associated with extremely poor prognosis. The best treatment approach to acute stroke in this setting is unknown. We sought to determine clinical outcomes in patients with acute ischemic stroke attributable to ICA occlusion treated with intravenous (IV) systemic thrombolysis or intra-arterial endovascu...
متن کاملIs tracheostomy associated with better outcomes for patients requiring long-term mechanical ventilation?
OBJECTIVE To evaluate the effect of tracheostomy on intensive care unit (ICU) and in-hospital mortality for patients requiring prolonged (> 3 days) mechanical ventilation (MV). DESIGN, SETTING, AND PATIENTS We retrospectively reviewed the charts of all consecutive patients admitted to our 18-bed tertiary care ICU over 3 yrs (2002-2004) and who received prolonged MV. Outcomes of tracheostomize...
متن کاملIntravenous thrombolysis before endovascular therapy for large vessel strokes can lead to significantly higher hospital costs without improving outcomes
BACKGROUND Limited efficacy of IV recombinant tissue plasminogen activator (rt-PA) for large vessel occlusions (LVO) raises doubts about its utility prior to endovascular therapy. PURPOSE To compare outcomes and hospital costs for anterior circulation LVOs (middle cerebral artery, internal carotid artery terminus (ICA-T)) treated with either primary endovascular therapy alone (EV-Only) or bri...
متن کاملEarly arrival at the emergency department is associated with better collaterals, smaller established infarcts and better clinical outcomes with endovascular stroke therapy: SWIFT study.
BACKGROUND AND PURPOSE Increasing time from symptom onset to emergency department arrival may incur greater ischemic injury and decreased likelihood of good outcomes after acute stroke therapy. The impact of time may be assessed bythe extent of acute CT changes, status of collateral vessels, and clinical outcomes. METHODS The SOLITAIRE FR With the Intention For Thrombectomy (SWIFT) trial comp...
متن کاملStent-assisted endovascular thrombolysis versus intravenous thrombolysis in internal carotid artery dissection with tandem internal carotid and middle cerebral artery occlusion.
BACKGROUND AND PURPOSE Tandem internal carotid and middle cerebral artery occlusion independently predicts poor outcome after intravenous thrombolysis. Recanalization of internal carotid artery dissection by stent-assisted angioplasty has recently been proposed when anticoagulation fails to prevent a new ischemic event. We recently reported a case of tandem internal carotid and middle cerebral ...
